The checklist IRCC uses to decide whether your application is complete. Here is every box on it, who ticks it, and where the matching file goes in the PR Portal.

What IMM 5644 is
IMM 5644 is the Document Checklist: Application for a Permanent Resident Card or Permanent Resident Travel Document. The current edition is 12-2024 (December 2024). One form serves both applications: a shared list at the top, then a section for PR card applicants and a section for PRTD applicants.
The form is blunt about why it matters: IRCC returns your application if it is incomplete or if any supporting document you need to include is missing.
Online, IMM 5644 is a row in the PR Portal's Forms table: you download it there, tick the boxes that apply to you, and upload it back in the same row. The paper instructions (put the documents in the checklist's order, checklist on top) are for mailed applications.1
Two pages, two jobs. This page is the form itself: what each box means and when it applies. The PR card documents guide is the practical side: which file to scan, how to name it, and a tick list you can print.

Four items everyone includes
IRCC says
Include the following items with your application: the Application for a Permanent Resident Card or Permanent Resident Travel Document (IMM 5444), fully completed and signed; proof showing that you met the residency obligation in the past five years immediately before the application, or since becoming a PR if it has been less than five years; fees, with a copy of the receipt showing the amount paid; and this fully completed Document checklist (IMM 5644).2
You do
IMM 5444: online, you complete it digitally in the portal; there is nothing to sign or upload separately.
Residency proof: the documents from Appendix A of Guide 5445. Portal slot: Proof of Residency in Canada.
Fee receipt: the $50 receipt from IRCC's payment page, uploaded as proof of payment.
IMM 5644 itself: yes, the checklist lists itself. Upload it in its row of the Forms table.
Additional forms and supporting documents, if they apply to you: Use of a Representative (IMM 5476) or Authority to Release Personal Information to a Designated Individual (IMM 5475); documents supporting a request to be considered on humanitarian and compassionate grounds; and, for a legal change of name, a copy of the legal documentation proving it.2
You do
Representative: tick only if someone acts for you (IMM 5476) or may receive your information (IMM 5475). Most people tick neither.
Humanitarian and compassionate grounds: tick only if you are asking IRCC to keep your status despite being short of days. That is a case for a licensed consultant or lawyer.
Name change: tick only if your name changed since your COPR. Portal slot: Name-change legal docs.
Don't tick a box "just in case". Each box says you enclosed that item.
Applying for a PR card: photos following the Photograph Specifications for Permanent Resident Cards (online, one photo uploaded front and back); a copy of one primary identity document, such as a passport; and your PR card (to renew, a copy of your current card; to replace it online, a photo of your destroyed card; if it was lost, a copy of a police report or incident number, if applicable).2
You do
Photos: the front and back images go in the portal's Photos step.
Primary identity document: portal slot Passport or Other Primary ID Document.
Your PR card: portal slot PR Card (front and back). A first card has no card to copy.
Only if they apply: IRM 0002 (change of sex or gender identifier), IRM 0004 and IRM 0005 (reclaimed Indigenous names), and for an applicant under 18, a birth certificate, adoption order or guardianship document, or school records (slot Birth certificate - dependent).
Applying for a child?
The extra document for an applicant under 18, and who signs.
Applying for a permanent resident travel document: copies of all pages of the passports or travel documents used in the past five years immediately before the application, including your valid passport or travel document. If they apply to you: supporting documents based on country-specific requirements, and an authorization for travel for minors.2

Renewing or replacing a PR card from inside Canada? Leave all three PRTD boxes empty.
These boxes are for permanent residents outside Canada applying for a travel document to come home.

Tick it, then upload it back
IRCC says
Include the listed documents with your application. Check each box once you enclose the item.2
You do
Open the PDF you downloaded from the portal in Adobe Acrobat Reader on a computer; IRCC's form page asks for version 10 or later.
Tick each box that applies to you and nothing else, save, and upload the saved file in the IMM 5644 row.
Doing it yourself is free and takes a few minutes.
Or let the renewal pack tick it
The Files plan ($19 CAD, one time) ticks the boxes for your answers on your own copy, English or French edition, and never the PRTD ones. Everything ($49) adds the answer sheet. IRCC's $50 fee is separate.
You're set. Every box that applies ticked, nothing else, and the saved PDF uploaded in its row. Next: IMM 5444.
Common questions
Do I have to upload IMM 5644 with a PR card renewal?
Yes. The checklist includes itself as a required item. Online, download it from the Forms table in the PR Portal, tick it, and upload it back in the same row.
Where do I download IMM 5644?
From the Forms table inside the PR Portal, or from the IMM 5644 page on canada.ca. The current edition is 12-2024, in English (IMM 5644 E) and French (IMM 5644 F).
Do I tick the PRTD boxes when I renew my PR card?
No. The last three items (passports, country requirements, authorization for minors) are for permanent resident travel document applications only.
What happens if something on the checklist is missing?
The form says IRCC returns the application if it is incomplete or a required supporting document is missing.
Is there a PR card renewal checklist I can print?
Yes. The PR card renewal documents guide on this site has a free tick list you can print. It is for you; the portal still needs IRCC's own IMM 5644, ticked and uploaded.
